Understanding Back’s basic anatomy
The human Spine (backbone) is one of the strongest parts of the whole body. It comprises 24 vertebrae (bones), one above the other. Each bone is separated by a disc in-between & fused together with strong muscles and ligaments.
The spinal Cord is present inside these vertebrae, which protect the spinal Cord. Nerves come out from the spaces between the Spine & connect with the brain and the whole body.
The Spine is divided into 3 parts-
- Cervical Spine
- Thoracic Spine
- Lumbar Spine
With the passage of time, the whole structure, including bones, ligaments, joints & discs, grow weak.
What are the most common causes of Back Pain?
There are numerous causes & conditions that can lead to chronic Back Pain. The primary reasons can be classified into 2 categories.

Mechanical Causes
Spinal Column consists of many moving parts like Tendons, Ligaments, Muscles, Vertebrae & Intervertebral discs. When the source of Back Pain is due to the movement of any part of the Spinal Column, it's termed the Mechanical Cause.
Various forces like Stress, Strain & Gravitational force constantly impact the Spinal Cord. The Spinal Cord performs continuous movement to balance these forces and prevent injury. Sometimes, these movements result in multiple conditions, leading to back pain.

LSS (Lumbar Spinal Stenosis)
When the nerve root passages or spinal canal narrows, it results in excruciating pain due to nerve compression in the Lower Back. This condition, known as Lumbar Spinal Stenosis, is very common in older adults as bones & tissues degrade with the passage of time.
Excessive compressions lead to pain radiating to the legs, often called Sciatica.

Back/Neck Strain or Sprain
The Spinal Cord is held together with robust spinal ligaments. When these ligaments are overstretched or torn, it results in a Sprain.
On the contrary, Strain occurs when your Muscles or Tendons are torn or overstretched.
Strain & Sprain both result in swelling as the soft tissues are damaged. This swelling causes pain & stiffness, making the movement painful.

Vertebral Compression Fracture
VCF or Vertebral Compression Fracture occurs when vertebral bone fractures due to a blunt force or trauma. The primary cause for such fractures is a fall or an accident. This results in excruciating pain.
This condition is more common among people with Osteoporosis (a condition in which the bones become fragile)

Spondylosis (Spinal Osteoarthritis)
It's a type of degenerative arthritis common amongst ageing individuals. As the body ages, the bones & the whole Spinal Column age. It can affect Neck, Mid Back or Lower Back. This causes vertebrae degeneration, affecting the Spine's joint facete.

Spondylolisthesis
This condition occurs when one vertebral bone slides over the lower bone beyond a specific limit. This leads to severe muscle pain, back pain & sciatica-like symptoms.
The more the upper vertebrae bone has slipped, the more is pain. The slippage is classified into 5 categories, viz. Grade 1 to Grade 5. Here, Grade 1 signifies minimal slippage, while Grade 5 indicates complete slippage.

Slouching
Prolonged hunching or slouching might feel relaxing to some, but this can cause severe back pain issues.
Staying Slouched for too long in a chair or while standing puts a lot of Strain on your soft & sensitive tissues. Over a longer period, this can result in Chronic back pain.

Lack of Exercise
When you stop exercising & moving your body, your back muscles weaken. So the next time you put some stress on your Back, this results in acute Back Pain.
Biomedical Causes
Apart from Mechanical causes, some bio-medical causes result in back pain. Here are some of the most common ones.
Disc Bulge:
Due to ageing, the intervertebral disc degenerates, and the cartilage becomes stiff. Due to this reason, the outer ring of the disc bulges out. This bulge compresses the nerves, resulting in a severe pain sensation.
Moreover, the body treats this bulged disk as an alien material. This activates the body's self-defence mechanism & the body sends WBCs to fight it. This results in excessive pressure on the nerve, resulting in more edema & pain.
Disc Herniation:
Each Intervertebral disc is made up of fibrocartilage that allows minimal vertebral segment motion. This disc has a gel-like material in the centre (nucleus pulposus), which is enclosed by the ring.
When this gel leaks from the disc due to the disc’s rupturing, it compresses nearby nerves. These compressed nerves become inflamed, causing irritation & pain. This is called Disc Herniation. Depending on which nerve is compressed, the pain radiates to that body part. It often includes arms & legs but can be at other parts too.
